Job Overview:

In this position, the APP performs an expanded nursing role by providing primary care to amyloid patients, which includes taking histories, making assessments, ordering lab tests, reviewing and reporting diagnostic lab values to the physician/s in accordance with Joint Commission, CMS and RWJUH guidelines, regulations and policies.

  • Coordinates clinical and administrative processes for patients through all phases of care management
  • Educate patient s families and staff regarding cardiac disease
  • Collaborate with physicians and other staff to evaluate and manage patients
  • Completes data capture analysis and reporting as well as other administrative functions required for cardiac amyloid program operations
  • The APN will participate in the care of both in-patients and out-patients

Qualifications:

  • Current Licensure as an Advanced Practice Nurse in the State of New Jersey OR
  • Current Licensure as an Physican Assistant in the State of New Jersey.
  • Current Licensure as an Registered Nurse in the State of New Jersey .
  • NCCPA Certification required
  • ACLS & BLS Certifications
  • DEA &CDS
  • Experience in the care and management of cardiac amyloidosis and other cardiomyopath is is required

Essential Functions:

  • The APP under the RWJUH Cardiovascular Service will practice within the inpatient and outpatient adult population,
  • Develops and implements a collaborative practice agreement specific to the clinical areas the APP will conduct their practice within and performs a periodic review of the collaborative agreement in accordance with hospital policy,
  • The APP will perform assessments of patients including history taking and physical examination prior to prescribing, based upon the individual needs of patients in accordance with the rights and privileges granted through RN licensure and APN certification,
  • The nature and extent of assessments performed, and resultant diagnosis made by APP'sfollow accepted standards of advanced nursing practice,
  • Facilitates the evaluation of cardiac amyloid patients, sarcoid and hypertrophic cardiomyopathy patients and other patients that may be assigned under his/her care,
  • Prepares case/s for presentation at weekly multidisciplinary meetings,
  • Initiates and performs selected therapeutic and diagnostic procedures, consultations and laboratory examination according to established protocol,
  • Maintains fiscal responsibility in completing patient evaluation and coordinates with financial counselor for insurance clearance,
  • Maintains and ensures communication with the patient s referring physicians and other health care providers,
  • Interfaces with families to obtain and provide information concerning patient's medical care and progress,
  • Reinforces education and follow up requirements and compliance with medical regimen including medications,
  • Assures follow-up testing,
  • Provides required information to databases as required,
  • Arrange patient admission and transfer,
  • Performs medical history documentation and physical examination,
  • Performs daily rounds, independently and with physicians and performs needed clinical tasks including progress notes, orders, dressing changes, and other care as needed,
  • Collects relevant data and prepares patient's discharge packet including needed prescriptions,
  • Coordinates appropriate referrals to specialties for patient care, Example: consultation with surgeon,
  • Write discharge summaries; and/or comprehensive discharge letter to referring physicians,
  • Arranges patient transfer between all levels of care,
  • Prescribes or recommends drugs or other forms of treatment or related therapeutic procedures,
  • Maintains knowledge of inpatient plan of care and responds to patient s needs,
  • Collaborates with multidisciplinary team and patient to coordinate discharge,
  • Review metrics designated by the department with the goals of improving patient outcome and supporting safety,
  • May participate in departmental research
