Job Overview:

This position enhances patient care and the operation of the Case Management Department by arranging specific services for patients, assisting in organizing and maintaining department resource files and provides assistance to professional associates of the Case Management Department. Assists in the delivery of the Important Message from Medicare (IMM) letter to patient or family as directed by the Case Management team. Assists with the distribution of daily assignment as determined by the CM Leader. Assists the CM team with delivery of bedside Durable Medical Equipment and clothing as needed for a safe patient discharge.

Qualifications:

Required:

  • High School Diploma required; Associate s degree preferred.
  • Minimum of one year experience in a healthcare setting.
  • Familiar with and able to use medical terminology effectively.

Scheduling Requirements (non-exempt positions):

Essential Functions:

  • Arranges for discharge transportation to home and to the next site of care as communicated by the Case Management team. Updates Case Management team to inform patients/families of any possible financial responsibility.
  • Arranges patient transfer to other health care facilities; confirms the appropriate means of transport with Case Management team and nursing staff; arranges with transport companies; and informs patient and/or family of arrangements.
  • Generates reports and conducts reviews in the electronic health record and in the referral management system as directed by Leadership. Assists in the daily operations of the Department as directed by Leadership.
  • Assists with providing patient information to post-acute next site of care or agencies as directed by Leadership and case management team for discharge planning.
  • Issues Important Message from Medicare (IMM) letter to patient or family as directed by the Case Management team in compliance with regulatory guidelines.
  • Conducts bedside deliveries of Durable Medical Equipment (DME), clothing, and any patient item needed during the stay and for discharge as communicated by the Case Management team.
  • Prepares and enters initial referral information to facilities identified by the Case Management Team as early as possible as part of the discharge planning process.
  • Updates referral information at the direction of the Case Management Team.
  • Upon request of the case management team, transfers demographic and basic medical information as part of the home health referral process.
  • Performs other duties as assigned.
