Job Overview:

Chinese Medical Program Navigator will be responsible for linking patients to hospital-based navigation services with the goal of improving individualized patient care coordination and increasing access across the health care continuum for the Chinese patient population. The Navigator will build relationships with Cooperman Barnabas Medical Center (CBMC), RWJBH affiliated, and non-affiliated providers to assist patients in the Chinese community with accessing CBMC s services.

The Navigator is also responsible for community outreach, events, communication, and education, whereby the Navigator will be key in instituting key contacts and networks among community organizations, senior centers, and local health agencies. The Patient Navigator will also assist as needed with other functions at the Florham Park Multispecialty practice.

Qualifications:

Required:

  • Bachelor s Degree in Related Field
  • Bilingual in English and Mandarin, additional Chinese dialect preferred
  • Experience in project and/or data management is required, preferably within the healthcare field
  • Demonstrated computer skills with Microsoft Office Suite

Scheduling Requirements:

  • Working Shift: Monday Friday
  • Day Shift
  • Full-Time, 37.5 Hours Per Week

Essential Functions:

  • Coordinate physician office visits, outpatient treatment, surgical and specialist referral appointments
  • Transportation arrangements, and accompany patients to physician offices and/or hospital departments to provide language support, with the overarching goal of supporting growth of Chinese Medical Program at CBMC
