Job Overview:

Responsible for the development, coordination, promotion, execution and presentation of Preventive Health Programs for Community members with the utilization of evidenced-based clinical information. Responsible for coordinating and completing various health screenings and programs for community members with timely and appropriate follow-up. Works collaboratively with MAP physicians, service line leaders, nutritionists and other health educators to develop and execute educational programs for the community.

Qualifications:

Required:

• Graduate of accredited School of Nursing
• Current RN license in the state of NJ

Preferred:

• Bilingual
• Bachelor's degree

Essential Functions:

• Demonstrates an understanding of needs of the community members and presents health programs focusing in on a variety of health issues by using clinically based information and networking with healthcare providers and outside organizations
• Perform ongoing evaluations of the programs, the instructors and participants to ensure the program meets the needs of the community with successful program satisfaction scores that consistently reach the departmental benchmark of 95 percent
• Works collaboratively with other health educators and healthcare providers to incorporate updated preventive health issues in programs
• Actively takes ownership in the development, implementation and presentation of these programs and becomes known as the expert in the community of Preventive Health topics
• Actively participates in major Community Health Special Events
• Maintains a flexible schedule with the ability to work evenings and weekends as needed
• Responsible to ensuring that there are a limited number of canceled programs per year due to low attendance
• Actively demonstrates the leadership role in educating family members in detail by reviewing health information and explaining results to screening participants
• Consistently attends Monday Morning Huddle and Staff Meetings, and contributes as a team member
• Works with PR Marketing when appropriate and creatively self-promotes programs to assist in increasing attendance
• Champions opportunities for self-directed continuing educational opportunities
• Attend training sessions/classes at the direction of the department head, and achieve certification that might be associated with the training
• Become trained for the Stanford Chronic Disease Self-Management Program CDSMP, and along with other department educators, run multiple sessions yearly of the CDSMP for our community and patient referrals.
• Works with community partners to provide valuable and necessary lectures, support groups, classes and screenings for our local community
• Assist in working on and at times management of Social Impact and Community Investment programs
• Involvement in the hospital CHNA, CHIP, and SIP community health improvement plans via programming and sitting on hospital committees related to these initiatives
• Attends twice-yearly MAP meetings to ensure continued connection and program planning with MAP physicians
