Nurse Practitioner (PT) Day Family Health Clinic, Hamilton

Job Overview:

RWJBH is seeking a Full Time Nurse Practitioner in the Family Health Clinic at Hamilton Hospital. The Nurse Practitioner is responsible for performing duties independently following established and standardized procedures and methods referring questionable situations to physician. The NP provides rapid response care through assessment, diagnosis, clinical judgment and management to restore, maintain and improve the health status of patients. In addition, the NP is responsible for reporting identified safety issues such as hazardous environments (i.e. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors.

Qualifications:

• Licensed RN
• Certification in Advanced Practice Nurse in the State of New Jersey required
• BCLS and ACLS required

Scheduling Requirements:

• Part Time
• Day Shift
• 20 Hours
• Variable days
• Flexible Shifts, no call, no weekends

Essential Functions:

• Obtains medical history and performs physical assessments appropriate to the patient's presenting complaints and age
• Analyzes, interprets and evaluates subjective and objective findings to develop an assessment in order to initiate a treatment plan or referral to appropriate physician or provider
• Performs routine therapeutic duties and collaborates/consults with the physicians as needed
• Reviews labs and radiology reports daily
• Develops a plan of care based upon assessment, which includes prescribing appropriate medications and treatments, providing follow up care
• evaluating the effectiveness of care
• Provides patient education regarding medications, treatments, anticipated outcomes and referrals
• Uses EMR to appropriately document patient visit information, while achieving the requirements of meaningful use
• Obtains appropriate continuing medical education and maintains licensure, national certification and NJ Board recognition as a Nurse Practitioner
