Job Summary:
The Palliative Care Advance Practice Provider will work in a collaborative and interdisciplinary environment to provide palliative care to seriously ill and medical complex patients on the palliative care program.
Job Description- (APN/NP):
Current Licensure as an Advanced Practice Nurse in the State of New Jersey.
CDS, DEA, BLS & ACLS Certifications
Required (Physician Assistant):
Current Licensure as a Physician Assistant in the State of New Jersey
CDS, DEA, BLS, ACLS Certifications
Scheduling Requirements:
Full time
Day Shift
37.5 hours/week
Responsibilities included but not limited to:
- Perform palliative care consultations as requested by the medical staff/referring providers based on established department protocols
- To conduct thorough assessments including but not limited to history and physical assessment
- Provide pain and non-pain symptom management
- Facilitate goals of care and advance care planning conversations
- Facilitate communication amongst healthcare team members and patients/families
- Establish a plan of care for palliative care patients based on the physical, emotional, psychosocial and spiritual needs of patients and their families
- Provide patient and family education regarding disease process, treatment, prognosis, symptom management, and end of life care
- Support the continuity of palliative care plans by assisting with discharge planning (in the role of a consultant)
- Educate healthcare professionals and hospital staff
- Perform accurate and timely billing of APN services
- Serve as a role model for palliative care delivery
- Establish collaborative relationships within and across departments to promote excellent patient care
- Demonstrate flexible and productive work habits and effective time management skills
- Participate in quality improvement initiatives
