Job Overview:

Qualifications:
Required:
- Registration in New Jersey as a pharmacist; current New Jersey licensure required
- Completion of course at an accredited School of Pharmacy, with a degree of Bachelor of Science in Pharmacy or PharmD
Preferred:
- Hospital pharmacy experience
Certifications and Licenses Required:
Scheduling Requirements:
- FT, 37.5 hours/week
- Rotated hospital holiday coverage and overnight coverage for vacations as appropriate
Essential Functions:
- The pharmacist is responsible for the provision of pharmaceutical care to all patient age groups including geriatric, adult, adolescent, pediatric and neonatal patients
- They are responsible for the verification and monitoring of both the prescribing and effectiveness of all medications and IV admixtures
- They exercise considerable independent decision making
- Verifies and monitors medication-prescribing orders by established drug references and established drug usage criteria, before dispensing, as defined by the Pharmacy and Therapeutics Committee or as determined by management, eg shortages, etc.
- Knowledgeable of changing policies, procedures, and clinical issues.
- Follows all procedures involved in the appropriate documentation of all medication errors, adverse drug reactions, and interventions and meets the required minimum documentation per employment status.
- The pharmacist appropriately enters and updates clinical interventions in the electronic health record system for all rejected orders.
- Verifies and monitors medical orders by established policies and procedures to meet regulatory requirements.
- Completes department and organization competencies and training as assigned.
