Pharmacist Clinical

Job Overview:

This is a Remote, Clinical Generalist Pharmacist role based within the Pharmacy for Cooperman Barnabas Medical Center,

Qualifications:

Required:

• Registration in New Jersey as a pharmacist; current New Jersey licensure required
• Completion of course at an accredited School of Pharmacy, with a degree of Bachelor of Science in Pharmacy or PharmD

Preferred:

• Hospital pharmacy experience

Certifications and Licenses Required:

• Licensed NJ Pharmacist

Scheduling Requirements:

• PT, 20 hours/week
• Monday-Friday 5p-9:00pm
• Rotated hospital holiday coverage and overnight coverage for vacations as appropriate

Essential Functions:

• The pharmacist is responsible for the provision of pharmaceutical care to all patient age groups including geriatric, adult, adolescent, pediatric and neonatal patients
• They are responsible for the verification and monitoring of both the prescribing and effectiveness of all medications and IV admixtures
• They exercise considerable independent decision making
• Verifies and monitors medication-prescribing orders by established drug references and established drug usage criteria, before dispensing, as defined by the Pharmacy and Therapeutics Committee or as determined by management, eg shortages, etc,
• Knowledgeable of changing policies, procedures, and clinical issues,
• Follows all procedures involved in the appropriate documentation of all medication errors, adverse drug reactions, and interventions and meets the required minimum documentation per employment status,
• The pharmacist appropriately enters and updates clinical interventions in the electronic health record system for all rejected orders,
• Verifies and monitors medical orders by established policies and procedures to meet regulatory requirements,
• Completes department and organization competencies and training as assigned,
