Job Overview:

Under the direction of the VP and supervision of the Director of the Community Specific Program, the Bilingual Patient Navigator will work with the multidisciplinary healthcare team to ensure smooth planning and execution of outreach activities. Will participate in institutional activities, attend community events, track the project status, analyze and abstract the data, perform data outcome analysis, and provide report/presentation to support the management needs.

Qualifications:

Required:

  • Bachelor degree required
  • Must be fluent in Chinese and English Bi-lingual, and able to read, write and speak Chinese proficiently

Preferred:

  • Experience in a health care setting

Essential Functions:

  • Assist Chinese speaking patients to verify eligibility in grant programs and schedule appropriate appointments. Make reminder calls to patients prior to screenings to ensure that appointments are kept. When follow up testing is required, serves as a navigator with MMC and other RWJBH hospitals to ensure appointment coordination
  • Organize patient schedules and manage their appointments to ensure services are received in a timely manner. Support teams at other sites and do navigations as needed, travel to different physician offices and hospital departments to provide on-site language support and follow-up appointment scheduling. Also communicate with site managers to continue the health care needs for all the patients
  • Support regional Chinese/Russian medical programs with community events planning, including webinar promotion, media & social media outreach for event promotion. Reach out to speakers, physicians, community leaders, venues regarding events-related needs. Build collaborative relationships with physicians, senior centers, weekend schools, supermarkets, community organizations, and hospital departments. Create and participate community events and activities. Writes Program event news, translate educational materials into Chinese and update advertisement with Chinese/Russian Media; Also responsible to communicate with vendors for event giveaways
  • Maintain organized paper and electronic document files when scheduling and following up on patient results. Maintain detailed database for the various programs/projects for abstracting/analyzing data to collect and identify community needs related to breast care prior to planning, implementing, monitoring, and evaluating programs designed to improve outcomes
  • Organize and conduct various health education programs in Chinese and English, providing translation when needed for speakers as well as maintaining databases, data analysis, mailing lists, and telephone networks to facilitate ongoing health programs
