Job Overview:

The Licensed Social Worker (LSW) will provide treatment, social work services and complex discharge planning on the Adult Psychiatric Inpatient Unit. This includes conducting psychosocial assessment, crisis-oriented individual treatment, family assessment and psychoeducation, group treatment, coordinating hospital to hospital transfers, and collaborating on service delivery with interdisciplinary team.

Qualifications:

Required:

  • Master's degree in Social Work
  • Excellent verbal and written communication skills
  • Capable of working with multiple tasks simultaneously
  • Expert critical thinking and problem-solving skills
  • Self-motivation
  • Organization and time management skills

Preferred:

  • Two years experience as a LSW

Certifications and Licenses Required:

  • NJ Licensed Social Worker (LSW)

Scheduling Requirements:

  • FT

Essential Functions:

  • Conduct biopsychosocial for all patients, engage families and/or community support services to assess patients' psychological, social, environmental and financial status and meet identified needs
  • Develop treatment plans with patients and our interdisciplinary team
  • Participate in interdisciplinary treatment team planning rounds to assess discharge planning needs, identify problems and communicate on-going interventions
  • Educate and engage patients and/or families about treatment, discharge planning, patient needs, as well as availability and eligibility criteria for programs/services benefits to meet those needs
  • Creatively negotiate, coordinate, and advocate community-based service to assure the highest level of needed support, continuation of care and wrap around services
  • Document in the Medical Record all assessments, reassessments and interventions to enhance multidisciplinary communications, as well as, to meet all regulatory and reimbursement standards
  • Attends appropriate Social Work conferences/workshops to maintain CEUs and license specific requirements
